About CenterWell Senior Primary Care

CenterWell Senior Primary Care is a network of senior-focused medical practices primarily located in underserved areas. It is one of the fastest-growing senior-focused, value-based care providers in the country, giving the 65+ community access to a personalized, care-team approach to healthcare.

CenterWell Senior Primary Care locations are staffed by board-certified physicians, nurses and behavioral health specialists, and patients will also have access to clinical pharmacists and social workers who have been specially trained to treat the senior population.

CenterWell providers have experience in both the treatment and management of most chronic and acute-care conditions. The centers also provide health education and value-added, well-being services on-site to help both patients and community members improve their health.

CenterWell facilities include activity centers offering everything from exercise classes to bingo and movie night – for all local seniors, not just patients. Many of our clinics are in underserved communities that continue to face barriers to care. CenterWell is working to help eliminate these barriers by making a visit to the doctor’s office convenient for the community.

How CenterWell Senior Primary Care helps Seniors

The Primary Care Organization of Humana, which includes CenterWell Senior Primary Care, serves more than 240,000 members from many different Medicare Advantage health plans, as well as some patients who have Original Medicare. In total, more than 220 centers in 11 states are part of our Primary Care Organization.

CenterWell Senior Primary Care has three clinics open in Phoenix, with six more opening by spring 2023.

CenterWell Senior Primary Care specializes in a care-team approach to healthcare. Patients have access to physicians, nurses, behavioral health specialists, clinical pharmacists and social workers who have been specially trained to treat the senior population…all in one location.

Preventative Care Focus

CenterWell’s care model focuses on preventative care to ensure seniors can live their best, healthiest lives. Appointment times are typically double those at traditional doctors’ offices – up to 40 minutes (or more, if needed) for each patient.

CenterWell knows that seniors may be living with multiple chronic conditions and need time to understand those conditions and their treatment to remain healthy and active.

Accessible Care Close By

CenterWell Senior Primary Care centers strive to be easily accessible. Many of our locations are in underserved communities with barriers to care, and our goal is to break down those barriers and make visits to the doctor convenient for the community. Many centers are in strip malls with large, easy-to-access parking lots and along bus routes.
