Angels Care Home Health

Angels Care Home Health is a Medicare-Certified Home Health agency that serves patients who have both common and more complex chronic conditions in the comfort of their own home or place of residence. Our team of health care professionals include skilled nurses, occupational and physical therapists, speech-language pathologists, medical social workers, and home health aides. Patients who typically receive home health service require skilled nursing to manage chronic conditions, such as, but not limited to Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), diabetes, dementia, or hypertension. Additionally, a patient may be recovering from an illness, injury, or surgery such as a hip replacement.

At Angels Care, every patient is carefully assessed, which allows us to provide therapy and skilled nursing care that is tailored for each patient. We admit patients and answer calls 24 hours a day, every day and all medical services are administered under the direction of our patients’ physician/non-physician practitioner (NPP-includes a nurse practitioner, clinical nurse specialist or physician assistant, working in accordance with State Law), who orders the necessary care. Our goals align entirely with those of the patient, family and provider and we maintain direct and frequent communication with the physician/NPP throughout the patient’s episode of care.
