About Angels Care Home Health

Angels Care Home Health is a Medicare-Certified Home Health agency that serves patients who have both common and more complex chronic conditions in the comfort of their own home or place of residence. Our team of health care professionals include skilled nurses, occupational and physical therapists, speech-language pathologists, medical social workers, and home health aides. Patients who typically receive home health service require skilled nursing to manage chronic conditions, such as, but not limited to Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), diabetes, dementia, or hypertension. Additionally, a patient may be recovering from an illness, injury, or surgery such as a hip replacement.

At Angels Care, every patient is carefully assessed, which allows us to provide therapy and skilled nursing care that is tailored for each patient. We admit patients and answer calls 24 hours a day, every day and all medical services are administered under the direction of our patients’ physician/non-physician practitioner (NPP-includes a nurse practitioner, clinical nurse specialist or physician assistant, working in accordance with State Law), who orders the necessary care. Our goals align entirely with those of the patient, family and provider and we maintain direct and frequent communication with the physician/NPP throughout the patient’s episode of care.

In addition, we also provide a Caregiver Support Program that empowers patients and caregivers with a realistic knowledge and greater acceptance of the various diseases and conditions most important to them. Most families are unprepared for the drastic changes in their life brought on by various diseases and conditions. Each of our caregiver support presentations addresses a wide range of challenges common to families coping with chronic diseases.

Our exclusive community classrooms and support groups are held in a variety of locations not limited to but including skilled nursing facilities, assisted & independent living communities & senior centers and are attended by one of our qualified clinicians.

What Clients Say about Angels Care Home Health

“I have received home health care from Angels Care Home Health for schizophrenia and mental health challenges. I needed help that I did not receive until I got connected with Angels Care. Before home health care, I was not able to stick up for myself. Now I can, and I am able to focus on me instead of others. Angels Care has helped me grow and be strong. I did not expect my Angels Care nurse, Lynn, to be so loving and caring. With Angels Care, I am getting along well with my mother, and we better understand each other’s feelings. The most helpful thing about home health care has been developing coping skills. Angels Care has helped me achieve my goals.”
Tony Tabor, Angels Care Home Health Patient
“I’ve had back pain for most of my life, and it got worse over the last 10 years. I’ve tried the pain clinic and had nerves burned. I’m surprised at how well home health care from Angels Care has worked compared to other treatments I’ve used. I wasn’t aware that you could bring in equipment, like the anodyne machine and ultrasound, and do physical therapy in your home. I am able to do more daily activities with minimal pain. I went from laying around most days to being more active. My pain level was at an 8 before and now it’s at a 1 or 2. I’m able to hit golf balls, which I wasn’t able to do for a long time. It’s been helpful to be more active and help more around the house with remodeling projects, and I’m able to help out my family more. I let everyone know how professional and caring Angels Care is. I would recommend anyone with back pain try the anodyne therapy and ultrasound.”
Simon Danborg, Angels Care Home Health Patient
“My doctor’s medical diagnosis was a deep, open, infected blister. Because of the wound’s location, I was unable to inspect it, clean it, apply appropriate antibiotics, or bandage it. My wife was unable to help because she just had breast cancer surgery. We needed help. Every Angels Care team member has helped in a caring way while adhering strictly to my doctor’s plan of care. The nurse calls my medical doctor or wound care doctor to discuss my specific needs, and the most helpful thing to my family is the way the Angels Care team keeps my wife involved in my progress. The nurse goes the extra mile in helping me in a caring way. They treat me as a real person, not as a number or just another patient. Angels Care has helped to heal my wound, given me peace of mind, and changed my life.”
